如果你 不 have health insurance, Medicaid or Medicare, you will be asked to bring the following documents to your first appointment at 英雄联盟外围下注 once per year:

  2. 居住证明 – This document must be no more than 90 days old with your name, address and a date on it. The following documents are approved for proof of residence:

  • Don’t wait until you are very sick or out of 药物治疗 to call your doctor.
  • Have your old medical records sent to your new doctor’s office, if it is your first visit.
  • Write down your health goals, problems, questions and concerns, and bring the list to your visit.

  • Health improvements that you want to make

  • Your personal and family medical history
